Student Work Permits

Student Work Permits can be requested on the following days:

Monday - Wednesdays

9:00 AM -2:00 PM

CLOSED on Thursdays and Fridays

The Office remains open to continue to support students seeking General Work Permits. Student General Work Permits may NOT be issued electronically due to the sensitive nature of Personal Identifiable Information (PII). Students seeking a General Work Permit may obtain the request form and the work permit at our Main Office. However, the student MUST present the following:

  • Students will need to communicate via email with their Counselor or Principal at the school site to obtain written Authorization for a work permit. Work permits will not be issued to a student without permission. 
  • Statement of Intent to Employ a Minor and Request for a Work Permit - Certificate of Age B1-1 form (original signatures from the student, parent, and employer). Students should NOT email this form to and from the employer. You are jeopardizing your personal information being exposed to hackers. This form can be picked up at the office if needed. 
  • Photo ID (Government ID, Valid Passport, or Photo School ID: must include the date of birth). The Office will confirm this through the student birth certificate or passport on file.  

Once all of these steps have been completed, the work permit will be issued. The minor (student) MUST be present to sign the work permit before it may be issued.
